Riordan died from blunt impact trauma of the head, neck and torso, a spokesman for the Philadelphia Department of Public Health said.

The Federal Aviation Administration on Wednesday ordered inspections of the type of engine that exploded Tuesday on Southwest Airlines flight 1380.

The FAA says that the coming AD will require ultrasonic, rather than just visual, inspection of engine fan blades after a to-be-defined number of takeoffs and landings.

From investigators' initial findings, the accident appeared remarkably similar to a failure on another Southwest plane two years ago - an event that led the engine manufacturer and regulators to push for ultrasonic inspections of fan blades on engines like the one that blew apart at 10,000 metres over Pennsylvania on Tuesday.

Despite Capt. Shults's heroic actions, shrapnel from the explosion shattered one passenger window, creating a vacuum between the pressurized cabin and the high altitude outdoors.

Pilot Tammie Jo Shults, who has been hailed as a hero for staying calm throughout the horrifying ordeal and successfully landing the plane, released a statement along with the flight's first officer, Darren Ellisor, on Wednesday. "We are not conducting media interviews and we ask that the public and the media respect our focus", the statement said.

The bank executive and mother of two from Albuquerque, New Mexico, was sitting directly next to the window and wearing her seatbelt, National Transportation Safety Board chairman Robert Sumwalt told CBS Philadelphia. "We are saying this is an engine failure", Sumwalt said. Since 2000, there have been 17 incidents involving Boeing 737-700 planes, eight of which involved Southwest Airlines flights. Sumwalt said a typical plane landed at around 250kmh.

"To be able to extrapolate that to the entire fleet, I'm not willing to do that right now".

"We value you as our customer and hope you will allow us another opportunity to restore your confidence in Southwest as the airline you can count on for your travel needs", the letter said, according to recipient Kamau Siwatu.

The jet's CFM56-7B engines were made by CFM International, jointly owned by General Electric and Safran Aircraft Engines of France.

A source familiar with the process, speaking on condition of anonymity, said the FAA expects to issue the finalized airworthiness directive soon.

Federal authorities are still investigating why the engine's fan blade snapped off mid-air, focusing particularly on how an interior crack on the blade prompted the incident. "Two wonderful men. they managed to get her back inside the plane, and we laid her down and we started CPR".

Riordan had dedicated her life to philanthropy, helping others in Albuquerque and the Southwest region, colleagues said.

After landing the plane with 149 people aboard, passengers praised the flight team who got them safely on the ground. During the incident, he logged on to the in-flight Wi-Fi to send messages to his family.

The flight, carrying 149 passengers on a domestic USA flight, made an emergency landing, but videos and images taken on board have caused alarm because many passengers appear to be using their emergency breathing masks incorrectly.

Jennifer Riordan was identified as the woman killed on the Southwest Airlines flight.
